Hazlewood and Mitchell Starc took the new ball in the second innings after the Men in Yellow had built a 95-run advantage with the bat.
The pacer struck gold with his first delivery, removing Tagenarine Chanderpaul for a golden duck.
He went on to win the prized scalp of opposition skipper Kraigg Brathwaite, thanks to a superb catch by Travis Head at short leg.
Hazlewood’s destruction continued as he eliminated Alick Athanaze and Kavem Hodge to finish with an impressive 5-4-2-4 record.
Hazlewood is still going strong in the final stages of his first stint, and he might take five wickets before his first break.
The pacer also had a successful home series against Pakistan, taking 11 wickets in three Tests, including a four-wicket haul in the last innings at Sydney Cricket Ground.
He just took his 250th Test wicket after removing Alick Athanaze in the first innings at Adelaide Oval.

West Indies, who struggled with the bat in the first innings as well, are now 54-5 after Cameron Green hit off his second delivery after coming into the attack.
Justin Greaves and Joshua da Silva are now sharing the crease, with skipper Pat Cummins also in the middle of his session following Hazlewood and Starc’s efforts with the new ball.
West Indies are still down by 40 runs and will need a strong combination from their last recognised batting tandem to keep the game going far into day three.
